In order to make sure the collection company you engage meets these requirements we have enclosed samples of the Duty of Care Waste Transfer Note you MUST receive every time you sell Failed Crowns & Bridges.

In addition Amalgam Waste is deemed Hazardous Waste and can only be uplifted from a Surgery with a Hazardous Consignment Note filled out in Triplicate.

!WARNING!

Failure by the Surgery to ensure that your collector is Licensed by the Environment Agency to handle this material could lead to the Consignee (Dental Office) being liable for not meeting their own due diligence and duty of care to ensure the material is removed in a safe and responsible manner.
